Ensocare and Dell Services to Provide Integrated Discharge Management and Care Transition Solution

OMAHA, Neb., April 14, 2016 - Ensocare announced today that it has partnered with Dell Services to enable end-to-end coordination of discharge management and care transition processes for hospitals. The cloud-based discharge management platform, powered by Ensocare, will help Dell customers improve bed utilization and patient experience while reducing readmissions.

“Now hospitals and healthcare organizations can benefit from a powerful integration of processes and services that couples the innovation and strength of Dell IT with the power of Ensocare’s automated discharge management platform,” said Ensocare President, Luis Castillo.

For many years decreasing the rate of hospital readmissions has been a high priority for U.S. health-care reform, and as such, more and more hospitals are being held accountable for patients’ outcomes post discharge. In fact, in 2015, 2,592 hospitals were penalized more than $420 million dollars for excess readmissions under the Hospital Readmissions Reduction Program.

Dell’s Population Health Solutions approach includes a complete portfolio of solutions, system integration, secure hosting, business process outsourcing and our continued deep commitment to the future of healthcare. Dell provides an efficient and effective solution for case management that seamlessly integrates with Ensocare’s no-cost, nationwide network of post-acute facilities. Using Ensocare’s technology solutions to replace once-manual processes enables health-care organizations to transition patients efficiently through care settings; monitor risk factors to allow for as-needed interventions; engage patients and family members by connecting them electronically to their care team, care plan and educational material; and capture and analyze data surrounding patient adherence, patterns of readmissions, provider performance and clinical outcomes.

Dell’s Healthcare Provider focus and strategy represents one of the longest standing components of the Dell Services business.

“Our Healthcare Provider business focuses on a set of core themes that we see as critical in supporting our provider customers,” said Sid Nair, vice president and global general manager at Dell Services for the Healthcare and Life Sciences (HCLS) business. “Some of our key core themes include Patient Engagement, Population Health and Revenue Cycle Services, all of which utilize Ensocare’s considerable strengths.”

About Ensocare
Ensocare is a cloud-based, Software-as-a-Service (SaaS) solution suite that automates the discharge process, effectively transitions patients between care settings and enables coordinated care across the continuum.  Automation is achieved through Ensocare’s Transition engine, which integrates with existing EHR systems to quickly match patients with the right post-acute facilities to meet their individual clinical and psychosocial needs. A multi-directional, secure exchange of information between the acute- and post-acute provider assures that patients’ clinical documentation and care plans travel with them, eliminating the potential for lapses in care, delayed or duplicative treatments. Ensocare also engages and connects a patient’s entire care team through its Sync and Connect engines, designed to enable continuous, anywhere-anytime communication, up-to-the-moment health data captured via wearables, apps and mobile devices, and by sending automatic alerts when risk levels indicate a need for intervention. Easy electronic access to patient education materials and the necessary community resources assure patients and family members have the information needed to manage and succeed post-discharge.  Finally, Ensocare’s Insight engine completes the software suite by capturing and analyzing rich data surrounding patient adherence, patterns of readmissions, provider performance and clinical outcomes.
